Insurance Information

Health 2 Home accepts most insurance plans and networks including Amerigroup, AmeriChoice, TennCare, Blue Cross, Aetna, Cigna, Medicare and many others. Please be aware that some plans may not cover all of our services and some services may require a physician's referral.

After your insurance has been billed you may still have an active balance. Depending upon your insurance plan and benefits, you may be responsible for additional charges for such as your deductible, co-insurance or services not covered by your medical benefits.


Please have your insurance card and proof of ID at the time of your visit. Co-payments are due at that time.

Self-Pay or Out-of-Network

If you haven't met your deductible or if you plan on paying for your visit out of pocket we offer affordable self-pay pricing for many of our services. If your visit is out-of-network we will provide you with a receipt so that you can file it with your insurance company for reimbursement.

Forms of Payment

We accept cash, credit and debit cards. Personal checks are not accepted.
